Step 1 – Your eligibility to enter an aged care facility?
Before you or your loved one can enter Ti Tree (or any other aged care facility), an Aged Care Assessment Team (ACAT) or Aged Care Assessment Service (ACAS), as it is known in Victoria, must assess you.
ACATs are teams of health professionals, such as doctors, nurses or social workers, who provide information, advice and assistance to older people who are having difficult living at home.
You can contact ACAT directly or be referred by your doctor, medical centre or local hospital.
An ACAT will then:
- Assess your individual needs taking into account your home life, medical history and support networks to work out the best care option for you
- Determine whether you are eligible for low or high level care
- Give you information about residential aged care and home care services in your area
- Help arrange special respite care should you require it
You will receive a copy of your assessment in person or by mail. If you are assessed as eligible you should keep a copy of this assessment, as it is the approval for your move to the residential aged care facility.

Step 2 – Deciding which aged care facility is right for you.
Once you or your loved one has been assessed by an ACAT as eligible for residential aged care, you may begin looking for a facility which suits your needs.

Step 3 – Establishing the costs.
The move to residential aged care can be a daunting and emotional process for you and your family.
At Ti Tree we are here to help you and your family understand the fee structure. We recommend a meeting with a member of Ti Tree's management to explain all the details.
Step 4 - Applying for residency.
If you or your family member decides to apply for residency at Ti Tree, we will provide an information pack containing the necessary Application for Residential Aged Care Forms.
Information about the applicant’s personal circumstances, legal and financial, is required. Applicants are invited to identify personal preferences so that we can most accurately assess the applicant’s needs.
When a place become available at Ti Tree the applicant or the applicant's nominated individual will be contacted. There is no obligation to accept the place. However, once an applicant accepts a place they will have up to seven days to move in.

Step 5 – Moving in to your new home at Ti Tree
Relocating to aged care can be an emotional and stressful experience for the resident, family members and carers.
When a loved one moves into an aged care facility there are many unknowns. They must adapt quickly to a strange new environment, a new daily routine and lots of new faces.
Ti Tree recognises that resident loneliness is an issue seldom addressed by aged care providers. At Ti Tree we are fortunate to have the 'Friends of TiTree' volunteer program providing company and support to Ti Tree residents from the outset, smoothing the early transition period. This is just one important difference of the Ti Tree experience.
Simultaneously our Ti Tree professionals we diligently to ensure new residents settle in as quickly as possible. TiTree Lodge provides virtually all requirements but a question often still asked is what to take to your new home? Our answer is very little, although it is common to bring personal memorabilia such as photos, books and favourite music.
